      目的通过GEO数据库和生物信息学分析,寻找食管鳞状细胞癌(esophageal squamous cell carcinoma,ESCC)潜在的血清学标志物,并探讨其在ESCC筛查诊断中的应用价值。方法通过GEO数据库(GSE38129)对ESCC患者癌组织和癌旁组织各30例样本进行分析,筛选出表达差异最明显的mRNA-基质金属蛋白酶1(matrix metalloproteinase 1,MMP1),并进一步通过在线基因表达谱分析工具(gene expression profiling interactive analysis,GEPIA)验证MMP1在癌组织和癌旁组织(para-cancerous tissues,PCT)的表达差异和预后的相关性。随后采用电化学发光免疫分析仪检测食管癌患者(109例)及健康人群(110例)血清样本中鳞状细胞癌抗原(squamous cell carcinoma antigen,SCC)、癌胚抗原(carcinoembryonic antigen,CEA)、细胞角蛋白19片段(cytokeratin 19 fragment antigen 21-1,CYFRA21-1)的浓度,同时双抗体夹心酶联免疫吸附法检测血清MMP1浓度。结果在GEO数据库(GSE38129)中MMP1的表达差异最为明显(logFC=4.849,P=7.90E-12)。在GEPIA数据库182例癌组织和286例癌旁样本中MMP1表达也有差异(P<0.05),生存分析显示MMP1与ESCA的总体生存率(Logrank P=0.4)和无瘤生存率(Logrank P=0.73)无关。ESCC患者血清MMP1、CEA、CYFRA21-1和SCC浓度均明显高于对照组的健康体检者(P<0.05);ESCC患者血清MMP1、CEA、CYFRA21-1和SCC浓度均随肿瘤分期升高而升高,呈正相关;血清MMP1和SCC联合检测ESCC的诊断灵敏度和特异性分别为82.13%和76.80%。结论外周血MMP1和SCC联合检测能显著提高ESCC的灵敏度与特异性,有利于ESCC的筛查诊断。

      ObjectiveThrough GEO database and bioinformatics analysis, potential serological markers of esophageal squamous cell carcinoma (ESCC) were searched, and evaluated the value of it as a marker screening and diagnosis of ESCC. Methods30 cases of esophageal squamous cell carcinoma and 30 cases of adjacent cancer were screened by GEO database (GSE38129). The most significant difference mRNA of MMP1 was screened. The expression of MMP1 in esophageal squamous cell carcinoma was further compared by Gene Expression Profiling Interactive Analysis (GEPIA) tool. Finally, SCC, CEA and CYFRA21-1 in serum samples of 109 patients with esophageal cancer and 110 healthy people were detected by electrochemiluminescence immunoassay. The serum concentration of MMP1 was detected by double antibody sandwich enzyme-linked immunosorbent assay. ResultsThe expression of MMP1 in GEO database (GSE38129) was the most significant difference (logFC=4.849, P=7.90E-12). The expression of MMP1 in 182 cancers and 286 adjacent cancerous samples in GEPIA database was different (P<0.05). Survival analysis showed that MMP1 was not associated with Overall Survival rate (Logrank P=0.4) and Disease Free Survival (Logrank P=0.73) of ESCC. The serum levels of MMP1, CEA, CYFRA21-1 and SCC in ESCC patients were significantly higher than those in the control group (P<0.05). The serum levels of MMP1, CEA, CYFRA21-1 and SCC in ESCC patients increased with the increased tumor stage, and showed a positive correlation. The combined detection of MMP1 and SCC could make the diagnostic sensitivity and specificity of ESCC reach 82.13% and 76.80%, respectively. ConclusionThe combined detection of MMP1 and SCC in peripheral blood can significantly improve the sensitivity and specificity of diagnosis of esophageal cancer, which is conducive to the diagnosis and treatment of ESCC.
